Who can do this:

  • Anyone with editing permissions for Forms, Homescreen, and Messages

Allow users to submit information conveniently through a form. For example, if preparing for an event, include questions like:

  • Dietary preferences or menu selections
  • Allergy information
  • Interest in a contest (e.g., upload a photo)

Add a form

Start by adding a form.

  1. Go to Forms ➤ Forms and click Add.
  2. Enter a Form title (e.g., Company Barbecue Attendee form).
  3. Click Add field.

Add fields

Add the fields below.

Prompt

  • Field type: Select Prompt.
  • Text: Enter instructions (e.g., We're looking forward to seeing you at our company barbecue! If you still plan to attend, please send us the info below by June 30th.)

Radio group

  • Field type: Select Radio Group.
  • Label: Enter text (e.g., Are you bringing a guest?).
  • Required: Tick to require an answer.
  • Options: Enter Yes.
  • Add Option: Select this and enter No.

Picker

  • Field type: Select Picker.
  • Label: Enter text (e.g., Protein).
  • Prompt: Tick to enter instructions (e.g., Please make your protein selection.).
  • Required: Tick to require an answer.
  • Option: Enter an option (e.g., Chicken)
  • Add option: Select this to provide additional options, as needed.

Click the copy (+) sign on the right of the field, to add another Picker question.

Text

  • Field type: Select Text.
  • Label: Enter text (e.g., Food allergies).
  • Prompt: Tick to enter instructions (e.g., If you or your guests have food allergies, please indicate them below.).

Media upload

  • Field type: Select Media upload.
  • Label: Enter text (e.g., Contest photo).
  • Required: Leave unchecked.
  • Media type: Select Photo.

Complete setup

Customize the form options:

  • Completion message: Enter a closing note (e.g., Thank you).
  • Completion image: (Optional) Select Upload to include an image.
  • Allow change/re-submit: Tick to allow respondents to edit their answers before the deadline.

Save and notify

Save and send a message that links to the form:

  1. In the bottom-right, select Save and send message.
  2. Choose recipients and a channel—then enter a brief message (e.g., Looking forward to seeing you at the company barbecue. Share your food preferences here).
  3. Click Send message.

Your message is sent and your form is active for the timeframe you set, or until you disable or remove it.

View responses

View & download responses:

  1. Go to Forms and select the form from the list.
    • The Form window opens and the Responses tab is selected by default.
  2. Select the List tab to view and download responses.
