Build a Form
Who can do this:
- Anyone with editing permission for Forms and Homescreen (if selecting Display tags and Availability)
Collect info or feedback from employees by creating a form that they can complete in the app or a web browser.
Add a form for users to complete.
To add a form:
1. Go to Forms ➤ Forms and click Add.
- A New form window opens.
2. Enter a Form title.
- This is automatically copied to the Menu title field.

3. Proceed below to Add fields.
When you add a field to your form, different options appear in the right panel, allowing you to select how the user interacts with that field.
To add fields:
1. Under Build your Form, click Add a field.
- Field type options appear in the right panel.

2. In the right panel, select a Field type from the dropdown.
- Depending on what you select, different options are generated.

3. Fill in the rest of the options:
- Text: Enter instructions or a description. Users don't interact with this field.
- Label: Enter a brief name that appears within the field box.
- Prompt: Include instructions, a question, or other text.
- Required: Require a response before the user can submit their form.
- Default value or selection: This appears within the field. For example, you can pre-select a checkbox.
- Add option: Use this to create a list of options.
- Format type: Choose the format, whether none, currency, or percent.
- Photo, video, or either: Select the type of media you wanted uploaded.
How do I format text?
The Parse Rich Text (advanced) options is available for use with the Prompt field type. This lets you apply formatting using Markdown. For more details, refer to this Markdown cheat sheet.
4. In the Build your form section, click Add field to add more questions. See Form fields.
5. Use the arrows, removal (x) or copy (+) symbols to the right of each field, if you wish.
6. Proceed to the steps below.
Finish filling in the form setup options.
To complete the form setup:
1. If you want, enter a Completion message and upload a Completion image.
- These appear to users upon submitting the form.
2. If applicable, select Allow change/re-submit.
- This let users revise and re-submit the form up until the form closes. If unselected, they can submit the form only once.

When should I allow users to revise and re-submit a form?
You might permit users to update their submissions before a final deadline, such as for a health benefits enrollment form.
2. Fill out the Menu appearance and Availability sections.
3. Select one:
- Save and send message: Opens the Compose page allowing you to send a message to selected recipients.
- Save and close: Shares your form for the selected timeframe, or until you disable or remove it.
