In addition to the standard [common sections](docId\:ceoUiF4_lCm4I65KxXKX5), the form options allow you to add different types of questions, give instructions, and more.

# Form title

Enter a heading in the **Form title** field. It's recommended to stay under 40 characters, even though the limit is 200.

# Build your Form

This is where you can add, remove, copy, rearrange, and edit the fields in your form.

**Add a field:** Select this to add a question or text. Use the panel on the right to set up the specifics for the question.

**Edit a field:** Select the field first, and in the right panel make the changes you want.

**Move, copy, or delete a field:&#x20;**&#x20;Select the field, and click the arrows, copy (+), or removal (x) signs that appear on the right.

# Field type

In the right panel, choose a **Field type** from the dropdown. Depending on which type you select, different options are generated:

| **Options**     | **Description**                                                                                                                                                                                 |
| --------------- | ----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- |
| **Text**        | When the **Prompt (text only)** field type is selected, enter instructions or a description. Users don't interact with this field.                                                              |
| **Label**       | This appears within the field box, on the right.                                                                                                                                                |
| **Required**    | (Optional) This requires a response, indicated by a dot.                                                                                                                                        |
| **Format type** | (Optional) Select this to choose **None**, **Currency**, or **Percent** in the dropdown. If you select **Currency** or **Percent**, a dollar or percent sign appears in front of the field box. |
| **Default**     | (Optional) Have a selection or value appear in the field box, prior to the user making a selection.                                                                                             |
| **Prompt**      | (Optional) Tick this to enter instructions, description, or a question.                                                                                                                         |

**When should I use** **Show advanced options?**

When this is selected, the **Field ID** is auto-generated and used for custom code, scripts, or integrations. If you're not using these, you don’t need to worry—it works automatically in the background.

### Text input

Add a field for users to enter text or a number. Choose from additional options:

- **Prompt:&#x20;**&#x41;dd text before the field (e.g., an open-ended question).
- **Multiline:&#x20;**&#x45;xpand the text box so users can see their response more easily.

### Checkbox

Add a checkbox the user needs to tick to agree to something. If the checkbox is not selected, the answer is calculated as no.

### Picker

Add a dropdown to offer multiple choices, allowing users to select one option.

### Picker w/ search

When you have a long list of options, add a dropdown where users can type into the search field. This helps users quickly find the option they're looking for. Users may choose only one.

### Address search

Request an address from users. Add a prompt and mark as required, if needed.&#x20;

Keep **Single line address** selected if you want the entire address populated into the field as a single line. If unselected, only the street address will populate this field. Then, to receive the complete address, ensure your form also includes these **Text input** fields:

- City
- State
- Zip code

### Radio group

Add a horizontal list of options, where users can select just one. Ideal for when you want to present clear, mutually exclusive choices.

- Date+Time picker

### Prompt (Text only)&#x20;

Add text that you want the user to read, such as instructions or a description. This is the only field that does not require the user to enter info or make a selection.&#x20;

### Signature

Use this field to request a signature. Add a label and mark it as required, if needed.

### User self-tagging

Let users select their own user tag(s), which will be automatically assigned to their profile. Any tags for existing categories will be updated, and new tags will be added.

Choose the following:

- **Tag category:&#x20;**&#x59;ou need to have [assigned a category](docId\:N6V-vlNUotdZgJpjx3qHt) in the ID field for each User tag (e.g., **Department\_**&#x4D;arketing).&#x20;
- **Selection mode:&#x20;**&#x4C;et users select multiple or only one.
- **Display mode:&#x20;**&#x54;ag cloud, list, or dropdown.
- **Sort method:&#x20;**&#x44;efault, alphabetical, or reverse alphabetical.

# Completion options

Finalize the form setup using the options below.

**Completion message:&#x20;**&#x49;nclude customized message (e.g., Thanks for your response!) that appears after users submit the form, or keep the auto-filled message. The character limit is 300.

**Completion image (URL):&#x20;**&#x49;nclude an image with your completion message.

**Redirect after completion:&#x20;**&#x43;hoose where users are directed after submitting the form.&#x20;

- **None:** Show the completion page with your message and image.
- **Homescreen:** Bring users to your app's main menu, either instantly or after a 5-second countdown with a message.
- **Other:** Bring users to specific app content.

# Reset options

Without using the next two options, users have only one chance to submit the form.

**Allow manual reset:&#x20;**&#x55;sers can update and resubmit the form anytime. Each new submission replaces the previous one.

**Auto re-set on a schedule:&#x20;**&#x55;sers can submit the form only once per the chosen frequency (e.g., daily or weekly). Each new submission will replace the previous one, and the form will automatically reset based on the selected schedule.&#x20;

# Notifications&#x20;

Use this option to instantly notify users via email, when each form is submitted:

**Notify users when this form is submitted:** Tick this and select one or more users.

&#x20;The email includes a link, which takes you to the forms list in the dashboard, shown below.
